SB 205 STORMWATER DISCHARGE COMPLIANCE. Any business required to obtain a stormwater permit under the Federal
Clean Water Act through enrollment with the National Pollutant Discharge Elimination System permit program MUST complete
the SB 205 Stormwater Discharge Compliance form. Failure to demonstrate required enrollment within 90 days of applying
can result in the denial of such business license application or renewal for insufficient information as required per State Law.

Under federal and state law, compliance with disability access laws is a serious and significant responsibility
that applies to all California building owners and tenants with buildings open to the public. You may obtain

A service fee for credit card transactions will assessed in the following manner:
• Transactions from $0.01 to $200.00 will be assessed a flat service fee of $2.75.
• Transactions from $200.01 and up will be assessed 2.4% percent of the total charges.
The service fee is non-refundable, regardless if the payment to which it relates is cancelled, refunded, credited or charged back.
